16:19:49 From Jeanette Boohene : will this be password protected like the Secretary of State site?? 16:19:54 From Althea Halchuck To Mary Drago(privately) : Will any advance directives other than the one the state offers, be involved with this system? Such as 5 Wishes or others? 16:22:22 From Mary Drago To Althea Halchuck(privately) : Has to match state law for advance care directives 16:22:32 From Sandy Severson : Will there be the ability to complete and update electronic healthcare directives on the site? 16:23:15 From Kathleen O'Connor : Is this service modeled after another state’s or country’s program of a similar type? 16:26:00 From Carla Sutter : If anyone has additional questions moving forward in regard to the registry please feel free to email me at Carla.sutter@healthcurrent.org 16:27:57 From Jeanette Boohene : will this presentation be available here in the archives for future reference to colleagues? When this goes live, need to market aggressively. So glad we are finally getting an accessible registry.....good job! 16:29:08 From Mary Drago : Yes, we have recorded the webinar and it will be on the website 16:33:45 From Vicki Buchda : Can you clarify that Five Wishes will be accepted by the Registry? It is my understanding that AZ does not specify that a certain form must be used. Multiple forms meet criteria. 16:34:05 From Althea Halchuck : How are updates or changes to the ACD achieved? 16:34:58 From Althea Halchuck : I also worry that only the state forms are being put into the registry. Many, many people use 5 Wishes because the state forms are so complicated. 16:36:30 From Althea Halchuck : There is a 2 witness statement on 5 Wishes but no notary is required in AZ. 16:37:31 From Carla Sutter : The Five Wishes is a legal advance directive and can meet the requirements for the State 16:37:47 From Althea Halchuck : www.AgingwithDignity.com is where 5 Wishes lives. There is a spot for notary so maybe if they notarize, it will work. 16:39:55 From Althea Halchuck : Thanks for the info!! 16:40:08 From Shannon D : thank you
